Description
The Provider Collaboration Manager serves as lead support for assigned provider value-based contracts which may include leading and facilitating negotiation, amending and maintaining agreements.
How you will make an impact:
- Obtains and analyzes financial metrics and performance.
- Maintains up to date knowledge on Advanced Medical Home program and Medicaid Transformation efforts.
- Supports and facilitates value based JOCs (Joint Operating Committee meetings).
- Develops and maintains contact database and disseminates communications to providers as necessary.
- Aggregates and analyze provider data form various sources to recommend and implement solutions to assist with provider collaboration activities.
- Encourages providers to act on health equity opportunities aligned with NC-DHHS programming.
- Partners with provider clinical liaison regarding quality metrics and performance as it relates to the delegated care management value-based agreement.
- Collaborates with other AMH teams across the health plan as needed ultimately cost of care savings.
- Promotes practice transformation towards meeting the goals specified by NC-DHHS.
- May participate in various community and multi-department committees to assure effective AMH program implementation as specified by NC-DHHS.
- Tracks progress of program and prepares status reports to management or senior management.
- Performs a range of contract related assignments such as reviewing care management payment reports.
- Continues to build knowledge of the organization, processes and customers.
Minimum Requirements:
Requires a BA/BS degree and a minimum of 3 years’ experience in data base maintenance related to health care; or any combination of education and experience, which would provide an equivalent background.
Preferred Skills, Capabilities and Experiences:
- Value-based contracting experience strongly preferred
- Knowledge of NC Medicaid Transformation strongly preferred
- Advanced Medical Home experience preferred
- Experience with Managed Care (Medicaid) preferred
- Experience with advocacy, public policy preferred
- Knowledge of social determinants of health and public health preferred
- Ability to travel to provider locations strongly preferred
