MDS Coordinator Responsibilities :

  • Complete and submit accurate MDS assessments in accordance with state and federal regulations.
  • Coordinate and participate in the interdisciplinary care planning process.
  • Review resident assessments and care plans to ensure they meet regulatory requirements and reflect resident needs.
  • Collaborate with nursing and therapy staff to gather assessment data and ensure comprehensive resident care planning.
  • Monitor changes in resident conditions and update assessments as needed.
  • Serve as a resource to staff regarding MDS coding and documentation guidelines.
  • Stay updated on changes in MDS regulations and requirements.

MDS Coordinator Qualifications :

  • Licensed Registered Nurse (RN) in the state of Pennsylvania.
  • Experience as an MDS Coordinator in a skilled nursing facility.
  • Responsible, ready, and eager to work
