Overview:
We are looking for a Care Manager AIM (Advanced Illness Management) RN - REMOTE to join our team. This position will directly report to the VP of Palliative Care and is responsible for working with patients, their families, PCP, IDT, and other health care professionals to provide and coordinate high-quality end of life care.
About You:- 2 years of prior skilled Registered Nurse experience
- Active acute unencumbered state RN license
- Must be willing and able to be licensed as an RN in all states applicable to role
- Ability and interest to provide direct patient care in homes and facilities
- Demonstrated experience developing and maintaining rapport with patients and families, and ability to work well with an interdisciplinary team
- CPR certification, copy of TB results
- Current driver license and auto liability coverage
