Job Title: Residential Case Manager
Reports To: Residential Clinical Supervisor
FLSA Status: Exempt
Compensation: $42,000 based on experience
Summary
As the Residential Case Manager, you will have the opportunity to be actively involved in the lives of youth ages 11-18, being part of a multidisciplinary team assisting youth to promote social, emotional, and intellectual development to return to a family setting in the community safely.

Key Responsibilities
- Provide/oversee case management services for all 29 youth in Residential Treatment Facility.
- Maintain appropriate correspondence with referral agencies, parents, funding agents, legal guardians and community resources to ensure ongoing communication and support surrounding treatment needs, including but not limited to Family Support Team Meetings, discharge/ transfer communication, Incident Reports, external referrals, resource connections, School needs, and medication needs.
- Actively coordinate multi-system care across various settings including Children’s Division, Court System, Department of Juvenile Justice, Schools, and other agencies involved in each case and participate as a member of the multi-disciplinary team to advocate for the mental health needs of the client.
- Act as School Liaison enrolling youth in school and actively communicating with School contacts on transitions, behavior, grades, IEPs and additional needs as they arrive.
- Monitor child/ family/ system participation and progress in engagement by completing and tracking monthly Visitor Pass forms and contact lists.
- Prepare required documentation and reports established by funding sources in timely manner.
- Attend necessary meetings including department, all-staff, supervision, in-service training, treatment teams, medical reviews, case reviews, court hearings and others as assigned.
- Facilitate weekly Life Skills groups to residents.
- Administer assessments throughout treatment to monitor youth’s progress, including but not limited to Eco Maps, TSCC, etc.
- Assist in collecting clinical information, admission documentation and submission of funding approval to the assigned funding source.
- Contact all referral sources as needed for updates on intake process and potential admission questions or concerns.
- Ensure the preparation of admission packet, communication of admission to the residential team and distribute necessary documentation.
- Complete admission process with client and guardian within the program,
- Enter and maintain program databases including physical and electronic charts as well as various system databases used for tracking and maintaining caseload information within a timely manner.
- Follow-up on obtaining any required documentation not received on the day of admission.
- Ensure completion of admission integrated assessment within 72 hours of admission.
- Participate in program meetings and other duties as assigned.
Education and Preferred Experience:
- Bachelor’s Degree from an accredited college or university preferred and/or five years of experience in the social service field required.

Job Knowledge, Skills and Abilities:
Essential:
- Demonstrated ability to utilize strength-based, therapeutic crisis intervention methods with adolescents.
- Understanding of at-risk youth client population.
- Ability to effectively instruct clients and support during the treatment process.
- Ability to read, interpret, and implement agency and program documents, including procedure manuals, treatment plans, etc.
- Excellent verbal/written communication and organizational skills.
- Interpersonal skills with the ability to collaborate with colleagues, community partners, families, etc.
- Computer skills: Microsoft Word and Outlook, and ETO database software.
- The physical requirements of this job include sitting, standing, walking and climbing stairs on a regular basis, lifting up to 100lbs, pulling and/or pushing on occasion.
- Most of the work for this position is performed under normal office conditions. The employee may need to travel from one location to another on an as needed basis.
