Baylor RN Hospice Case Manager
Responsibilities:
- Initial Start of Care and visits for Hospice agency
- Friday, Saturday, Sunday 32 hours. Full time 40 hour salary
- Coverage area includes parts of Hartford and New Haven counties
Requirements:
- A Registered Nurse with current Connecticut License
- 2 years clinical experience including acute care
- Home Care or Hospice experience preferred
- End-of-Life care experience preferred
- Compassion and integrity
- Associates or Bachelors Degree from an accredited college required.
Job Type: Full-time
Pay: $85,000.00 - $100,000.00 per year
