LPN Hospice (full-time, permanent hire position with excellent full benefits and competitive compensation/bonus structure!)
Responsibilities:
- Provide direct patient care according to the established Plan of Care
- Suggest changes in Plan of Care to Supervising RN in response to identified patient care concerns
- Provides accurate and timely documentation consistent with the Plan of Care.
- Communicate effectively with all Hospice team members, patients and families in order to achieve best patient outcomes
Requirements:
- Valid license as a Licensed Practical Nurse (LPN)
- Graduate of an accredited school of nursing
- Minimum 1 year of Hospice experience
- Valid Driver’s License, auto insurance and reliable transportation
- Excellent observation, problem solving and communication skills
- Compassion and integrity

Pay: $70,000.00 - $75,000.00 per year
