Home Care Registered Nurse Supervisor
Position Overview:
Everlasting Growth is currently looking for a Home Care RN Supervisor licensed in the state of Georgia to participate in the planning, development and implementation of the service plan to clients, perform home supervisory visits and assist with annual competency reviews of aides. Many of our clients (but not all) are children and adolescents under 21 years of age who have special health needs, are medically frail or medically compromised with a severe medical condition or developmental disabilities needing private home care assistance.
Shift Details:
GA - Multi-County
Intermittently (Sat/Sun as needed)
Requirements:
- Clean criminal background check
- Negative TB test
- Signed statement of history of no misconduct
- Valid Driver's License
- Basic Life Support (BLS)/CPR; First Aid Certificate
- Ability to Read and Write English fluently as essential to safety and effective function of job
- Leadership or Prior Supervisory Experience
- Possess cellular phone with access to internet
- Experience working in home health or patient home services preferred
Responsibilities Include:
Conduct Assessment to determine if a Client is Medically Fragile or Medically Compromised
Review Clients History and Draw a Conclusion on the Medical Needs of the Client, Provide Personal Service Plan for Each Client
Build Relationships, Ability to Take Verbal Orders and/or Allow Parents or Child’s Guardian to Be Involved in their Child’s Physician Ordered Service Plan of Treatment Along With Any Service Goals
Consult with Assigned Staff That Will Reach out Prior to Going Out to Serve the Client
Supervise/Train CNAs PCAs and Nurses, Supervise Management of Records, Medication Administration, Monitoring Medication
Provide assessment in annual competency reviews and supervisory evaluation and document staff outcomes
50% - Assess clients, participate in the development and implementation of each client’s service plan and assess the follow-up needs of those clients receiving nursing services. During the visit the RN Supervisor will make assessment of the client’s condition to determine if the current level of care is appropriate. This includes checking vital signs, review of progress notes, any problems, the appropriateness of the current level of services and the client’s satisfaction with services. If needed, the supervisor will make changes.
30% - 40% Perform minimum quarterly home supervisory visits to client’s home while services are being rendered by Certified Nursing Assistants, Licensed Practical Nurses and companion/sitters. Document visit on a Home Supervisory Form while aide must be present on some of these visits and time of performing services.
10% - 20% Conduct Orientation Training/Involvement in annual assessment of staff through direct observation or demonstration of the task the staff is assigned to provide and document the outcome of the assessment and provide staff a written report of the visit. Investigate incidents/grievances and investigate accidents
