Overview: Family Centers is seeking a caring, motivated, and well-organized Primary Care Registered Nurse (RN) to support us in delivering a healthcare experience that is of high quality, comprehensive, and compassionate for our patients. Family Centers Health Care, a FQHC (Federally Qualified Health Center) provides primary healthcare, dental, and mental health services to children and adults living in Greenwich and Stamford with an emphasis on the special population living in or near public housing.

Summary Of Responsibilities

  • The Primary Care Registered Nurse (RN) effectively collaborates with healthcare providers to guarantee the provision of safe and high-quality patient care, fostering excellent communication and prioritizing a timely and seamless patient experience.
  • Evaluates patient responses to care plans, collaborates with the integrated care team for personalized treatment adjustments, and educates patients/families on optimal home health care activities.
  • Provides nursing care during visits in alignment with provider orders, encompassing tasks such as medication administration, injections, and executing standing orders.
  • Registered Nurse plays a key role in coordinating chronic disease management, diligently following up on lab visits, hospital admissions, discharges, and specialty referral visits.
  • Assumes responsibility for managing nursing care, meticulously documenting assessments and interventions to uphold a comprehensive patient care approach.
  • Efficiently conducts triage activities for both scheduled and walk-in patients, addressing medical inquiries with expertise.
  • Executes various nursing procedures, including dressing changes, suture removal, oxygen administration, and basic wound care.
  • Demonstrates a keen ability to assess patient conditions and respond promptly in emergency situations.
  • Actively engages in ongoing Quality Assurance and Improvement initiatives, ensuring strict adherence to quality standards and regulations, contributing to infection control committee reports.
  • Proactively participates in training, consultations, and supervisions.
  • Effectively implements directives and advice from supervisors, training personnel, and coworkers, while also excelling in other assigned duties.

Education And Experience

  • Graduate of accredited school of nursing program
  • Holds a a current RN license in the State of CT
  • Current Cardiopulmonary Resuscitation (CPR) / Basic Life Support (BLS)
  • Highly detail-oriented with superior analytical skills
  • De-escalation and ability to address emergency events
  • Excellent customer service/ communication skills
  • Demonstrated ability to utilize computer technology, EMR and to willingness learn

Preferred

  • Experience in an ambulatory primary care setting, preferred
  • Bilingual (Spanish) is strongly preferred
  • Infectious Control Certified
  • EPIC experience

Physical Requirements

  • Must be able to stand for up to 8 hours a day
  • Must be able to bend without restrictions
  • Must be able to lift a minimum of 50 lbs.
  • Must have excellent hand/eye coordination

Salary commensurate with experience. Sign-on bonus may be available.
