**Job Summary:**
The Client Support Coordinator will provide habilitative assistance and care to clients in various Agency programs. This role involves direct daily care, teaching, and supervising of clients, along with record-keeping responsibilities.
**Responsibilities:**
Directly supervise clients, monitor their status and location at all times.
Assist in preparing and implementing individual client plans.
Teach, supervise, and assist clients in adult daily living activities.
Monitor client progress on goals and protect client rights.
Complete incident reports and medication error reports.
Implement behavioral plans and therapist recommendations.
Develop and implement recreation and community participation activities.
Collaborate with Clinical Director, Consultants, families, and other agencies.
Attend meetings, in-services, and required trainings.
Administer medications, provide routine health care, and first aid.
Maintain cleanliness and organization of program sites and equipment.
Transport clients to designated activities and programs.
Participate in emergency drills and maintain medication certification.
Keep accurate records of services provided to clients.
**Qualifications:**
Successful completion of Medication Certification Program.
Successful completion of PMT, CPR, and First Aid training programs.
Valid CT driver's license with a satisfactory driving record.
High School Diploma or equivalent.
Minimum of two years of experience working with individuals with developmental and/or behavioral challenges preferred, or equivalent combination of education and experience.
**Physical Demands:**
Regularly talk, hear, stand, walk, sit, use hands, and reach.
Lift and move up to 75 pounds and exert over 100 lbs. of force if needed for physical restraint procedures.
Specific vision abilities required, including close vision and ability to adjust focus.
**Language Skills:**
Ability to read and interpret English documents, write routine reports, and correspondence.
**Mathematical Skills:**
Ability to add, subtract, multiply, and divide using whole numbers, fractions, and decimals.
**Reasoning Ability:**
Apply common sense to carry out detailed instructions and deal with problems involving concrete variables.
