About this Role:

Establish a cooperative working relationship with families through home visits in accordance with the mission of FamilyCore. Identify needs and concerns and assist family in meeting them.

Responsibilities:

  • Manage and maintain a safe, clean, and efficient environment which complies with all Council on Accreditation standards and local, state and federal laws and regulatory bodies.
  • Provide direct counseling, case management and intake services to a varied caseload.
  • Conduct offsite services to promote positive familial relationships.
  • Establish and maintain collaboration with families.
  • Adhere and maintain all contract requirements.
  • Oversee the development and maintenance of procedures and distribution of quality documentation that complies with the requirements of accreditation, licensing, and contracts.
  • Working knowledge of de-escalation techniques and methods.
  • Adhere to standards adopted through Council on Accreditation.
  • Ability to organize and maintain data and confidential information.
  • Develop and maintain working relationships with all internal and external clients.
  • Collaborate and contribute to team and FamilyCore quality improvement and evaluation activities.
  • Ensure safety protocols and policies are practiced at all times.
  • Assume responsibility for professional development and cultural competency.
  • *Perform other duties as assigned.

Requirements:

Education and Experience:

  • Bachelor's degree from an accredited university in Social Work or related field required.
  • 1-3 years' experience required.

Competency:

  • Clinical experience to assess and support client assessments as needed.
  • Excellent planning and organizational skills.
  • Strong interpersonal and problem-solving skills.
  • Ability to establish and maintain a client focus.
  • Ability to set and maintain professional boundaries with clients.
  • Must pass required background check with Illinois Department of Children and Family Services.
  • Must pass background checks with Illinois Department of Public Health (IDPH) Health Care Worker Registry, and Illinois Department of Healthcare and Family Services OIG Provider Sanctions.
  • Ability to analyze, process, organize and maintain data and confidential information.
  • Must possess valid Illinois Driver's License in good standing and proof of valid vehicle insurance.

Competency:

  • Utilize time tracking software and all other software required to fulfill duties.
  • Safe, reliable personal vehicle, to be used for FamilyCore business, including transporting clients.
  • Must be CPR and First Aid certified.
  • Must complete child passenger safety training.
  • Ability to be sensitive and understanding of cultural and lifestyle differences of all people regardless of race, religion, sexual orientation, age, or background.
  • Successfully complete FamilyCore training as required.

Physical:

  • Frequent sitting lifting, pulling and pushing up to 15 lbs.
  • Regular walking, grasping, fingering, standing and other physical activities related to an office environment.
  • Occasional stooping, reaching overhead.
  • Operate general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer, with or without dual monitors.
  • Perform all job requirements with or without reasonable accommodation.

Work Environment:

  • Work occurs in and out of an office environment. Frequently exposed to loud noise and outdoor weather conditions.
  • May be subject to noise, distraction, and interruptions. May be subject to difficult client interactions that involve behavioral and/or emotional challenges.
