Overview:
The Supports Coordinator is primarily responsible for coordinating support services to persons with Intellectual Disabilities and/or Autism and their families/caregivers. Major functions involve locating, coordinating and monitoring supports for individuals.
Essential Duties and Responsibilities:
- All essential duties and responsibilities are completed within regulatory timeframes set forth by the Office of Development Programs (ODP).
- Develop Individual Supports Plan (ISP) for all assigned individuals on caseload.
- Ensure all participant ISPs are up to date and current.
- Complete and resubmit required corrections to all returned ISPs and pending revisions.
- Complete all required monitoring for each participant.
- Enter detailed monitoring documentation for each participant.
- Each Supports Coordinator will meet their specific productively goals per departmental policy.
- Ensure each participant’s Prioritization of Urgency of Need for Service (PUNS) form is updated annually and as needed.
- Complete timely and thorough service documentation/notes after each contact or service provided.
- Complete required minimum of 24 hours of annual training.
- Participate in all staff development, supervisions, trainings and meetings as required.
Requirements
- A bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science; or
- Two years’ experience as a County Social Service Aide 3 and two years of college level course work, which include at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service; or
- Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.
- A valid driver’s license along with access to a vehicle.
- Newly hired Supports Coordinators will successfully complete ODP required SC Orientation Curriculum.
- Management of work processes along with strong computer skills for documentation purposes.
- Ability to maintain confidential information within HIPAA guidelines and organizational policies.
- Ability to be flexible especially with scheduling and site locations while performing and prioritizing multiple functions and tasks.
- Ability to establish a respectful relationship with persons served to help them gain skills and confidence.
- Knowledge of the theories and principles used in providing supports coordination services.
Working Conditions:
- The work is in various settings including but not limited to Familylinks offices, the individual’s home, school, community setting or provider agencies.
- Must be flexible with schedule as some evenings and weekends may be required to work.
- Must be able to travel to all locations as assigned.
- Lifting up to 10 lbs. maximum and occasionally carrying such articles as files, ledgers and small tools.
- A limited amount of walking and standing may be necessary in carrying out duties
