Life Style Coach – Diabetes Prevention Program – Wyandotte/Southgate, MI
96 million Americans have prediabetes and our goal at First Mile Care is to halve that number by 2028. How? By scaling the CDC’s proven Diabetes Prevention Program (DPP). The DPP is a year-long lifestyle change program clinically proven to stop the onset of type 2 diabetes. We educate participants about how nutrition, fitness, sleep, exercise, and mindfulness combine to build healthy habits that lead to sustainable lifestyle change. We are committed to creating a nationwide network of First Mile Care coaches. We train local heroes by ZIP code to help their community members make the right decisions to gain control of their health and reverse prediabetes.
THE POSITION
Diabetes Prevention Program lifestyle coaches lead small groups of people with prediabetes or at high risk for developing type 2 diabetes through a year-long program clinically proven to reduce the risk of diabetes. Using a CDC-approved, evidence-based curriculum, coaches provide in-person support and guidance to participants that help them develop the skills needed to maintain healthy lifestyle habits.
We are looking for coaches who can offer on-site, in-persons in the greater Wyandotte and/or Southgate, MI area.
Coaches are responsible for:
- Facilitating weekly or bi-weekly sessions with a group of 8-15 participants through the year-long Diabetes Prevention Program.
- Providing curriculum to class participants.
- Maintaining accurate attendance logs and inputting data into the course management system in a timely manner.
- Assisting with participant recruitment activities.
REQUIRED QUALIFICATIONS
- Commit to a year-long in-person coaching position.
- Successful completion of the Diabetes Prevention Program training or successful completion of the DPP training through First Mile Care upon hire.
- Familiar and proficient with motivational interviewing and group facilitation.
- Have strong communication and relationship-building skills, the ability to build community within a group, and have empathy for health seekers.
- Be passionate about the First Mile Care mission in helping community members live a healthier lifestyle.
PREFERRED QUALIFICATIONS
- 2 years of experience in health education and/or coaching with individuals and groups for disease prevention, health promotion, and/or behavior change
- Knowledge of basic health, nutrition, and fitness guidelines.
Job Type: Part-time, Contract
Pay: From $22 per hour/ $45 per session facilitated, with attendance tracked
Schedule: Monday to Friday
