General Summary :
The Post-Acute Services Liaison serves as the key point of contact between patients, healthcare providers, and Fisher-Titus Post-Acute Services.
They establish eligibility for services, communicate with families, and interact with a wide range of staff members, from admissions coordinators to case managers to physicians.
They collaborate with hospitals, acute care, long-term acute care, hospice and rehabilitation facilities to ensure patients successful transition to the next appropriate level of care.
The Post-Acute Liaison will coordinate patient discharges to home health, skilled nursing, and assisted living. The will promote Fisher-Titus Post-Acute programs and services while maintaining the standards of practice consistent with quality healthcare and maximizing human, financial and equipment resources.
They will explain the types of care and services home health, assisted living, and skilled nursing offer to patients and their families while also coordinating with other hospital and agency staff members to ensure a seamless transition.
Will be responsible for networking in and around the community, attending public events, and representing Fisher-Titus which could include other aspects of marketing, like telecommunications or market research.
