Clinical Manager Diabetes/Health Education

POSITION SUMMARY:

The Manager is responsible for providing and improving continuity of care for members and improving communication between providers, staff, patients, and their families. The Manager will use a team approach to ensure appropriate management of all health education staff.  This position monitors ADA Self-Management Recognition Program, class calendar scheduling, and communication of class offerings to both internal and external customers. It is responsible for collaborating with other departments as it relates to diabetes and health education services/programs. This position will maximize management of data collected, to facilitate services/programs, improve continuity, quality, productivity, and cost effectiveness. 


 

QUALIFICATIONS:
Education, skills and experience: 
   


  • Bachelor’s Degree in Dietetics or Nursing.

  • Master’s degree preferred.

  • Registered Dietitian Nutritionist (R.D.N.) - Registered through Commission on Dietetic Registration  and current license from Department of Professional Regulation State of Florida or Registered Nurse (RN) with current valid Florida State Board of Nursing license  

  • Certified Diabetes Care and Education Specialist (CDCES) preferred.

  • Prior experience with leadership roles, team building, and leading in managed care setting or any health care facility required. 

  • At least 5 years’ experience in patient care setting. 

  • Experience with electronic health records and telehealth (including, but not limited to, Zoom and Microsoft Teams).

  • Ability to motivate, coach direct, mentor, evaluate personnel and be available by phone.

  • Knowledge of administrative functions of managing files, records, and using Microsoft products to include Word, Excel, PowerPoint, and Outlook.

  • Experience with inductive/deductive reasoning, problem solving, and motivational interviewing.

  • Knowledge of curriculum development, manual preparation, use of visual aids, course outlines, and other materials and methods used in teaching. 

  • Written and oral comprehension with the ability to listen, understand, and communicate information and ideas in writing and verbally.

  • Excellent interpersonal and organizational skills and be able to coordinate multiple needs and entities.

  • Must be able to multi-task, prioritize duties, and work independently or with others in a multi-disciplinary team environment.

  • Must be a team player with high priority on efficiency, quality and meeting the demands of departmental operations.

  • Works collaboratively with other departments in order to meet organizational goals.

  • Must have flexibility to travel for education classes at various times and FHCP locations as needed. 

  • Must have a valid Florida driver’s license; must have own car.
