What You Can Expect
- Ensure all clinic staff is practicing within their licensure, while also certifying all licensed clinical staff is current with licensure and compliant. Ensure sufficient Day Room staffing by PCAs and Clinic per staffing guidelines
- Communicate with the Center Manager on case management/homecare operations and issues
- Support clinic nurses with continuous process improvement for nursing plan and protocols utilizing EPIC, while reviewing the plans of other disciplines to collaboratively meet the participants' continuing care needs
- Demonstrate competency in clinical skills such as, physical assessment, intravenous therapies, symptom management, controlled substance management and participant relationships
- Consult, refer, and collaborate with other interdisciplinary team members or health care professionals to promote quality and continuity of participant care.
- Apprise IDT of changes in participant's condition, care needs, and/or concerns of participants
- Lead Polypharmacy calls with Providers and Contracted Providers
- Regularly communicates with nursing administration of contracted ALF and ECF facilities with participant updates and discusses any participant/staff issues requiring interventions by facility
- Bachelor's Degree in Nursing - Required
- 3 years Nursing/Patient Care - Required
- 1 year PACE - Required
- 1 year Supervisory experience - Required
- 1 year Ambulatory/Home Health - Preferred
- Basic Life Support Program (BLS) - American Heart Association -within 90 days of hire - Required
- Registered Nurse (RN) - Required
- Drivers License - Drivers License - Required
