What are the responsibilities and job description for the Counselor position at Freedom House?
Position Title: COUNSELOR
Reports To: Clinical Supervisor
Basic Function: Provides direct client services in keeping with the mission and program philosophy of Freedom House. Appropriately carries out the following Core Functions of Counseling: Assessment, Treatment Planning, Counseling, Case Management, Crisis Intervention, Client Education, Referral, Report and Record Keeping, Consultation with other professionals in regard to client treatment and services
Core Functions:
1. TREATMENT PLANNING: Assists the client and significant others in the development and implementation of the service plan.
2. REPORT AND RECORD KEEPING: Maintains appropriate and confidential records of client progress under the supervision of the Clinical Director.
3. CASE MANAGEMENT: Facilitates planning and problem solving meetings with clients, significant others, and collateral agencies when necessary, advocating for clients and assisting with gaining needed services.
4. COUNSELING: Provides individual, group, and supportive counseling.
5. CONSULTATION: Participates in case conference review sessions, and clinical supervision as directed by supervisory staff.
Additional Duties Related to Core Functions:
6. Works on a schedule as assigned by supervisory staff; may be required to fill-in in the event of illness or vacation; works holidays as needed; works rotating shifts that include evenings and weekends.
7. Teaches and supervises daily living skills which may include but are not limited to cooking, cleaning, laundry, banking, budgeting, personal hygiene, menu planning, shopping, etc.; assists and supervises clients with household maintenance issues.
8. Provides progress reports and other required status reports internally and externally as requested or required.
9. May accompany clients to appointments; may participate with clients in community activities and in therapeutic and/or vocational programs as needed to ensure service delivery.
10. On call for crisis management; contacts supervisor in the event of acute crisis.
11. Proficient in clinical documentation; maintains appropriate client files.
12. Utilizes NJSAMS software in accordance with policy; completes any and all modules as required, including extension requests for treatment that require a LOCI report.
13. May provide client transportation as needed.
14. Performs other duties as assigned or requested to further the mission of Freedom House, Inc.
