HealthCare Navigator/ Social Worker

Overview:

The GBS|CIDP Foundation is seeking a candidate for a position within the patient services team. The Healthcare Navigator will counsel patients & caregivers on their journey with the rare neurological conditions of GBS, CIDP, MMN or variant conditions. The Healthcare Navigator will be a self-starting, problem-solver, who enjoys utilizing their social work expertise in a variety of ways. Specific tasks include:

MEDICAL INSURANCE REIMBURSEMENT GUIDANCE: Provide solutions to inquiries regarding private and public healthcare insurance reimbursement.
PATIENT RESOURCES: One on One consulting with patients to provide resources for cases that may require individual patient navigation. This will include psychosocial and financial resources, social security disability, adaptive devices, transfer of care & discharge planning, and finding appropriate rehabilitative care.
COMMUNITY GROUPS: Lead online support group programs for various types of community members (ie. Women’s Group, Seniors, Care Givers)
COMMUNITY OUTREACH & DEI Initiatives: Attend 3– 4 health fairs per year, as a Foundation representative, in association with the Foundation’s Medical Centers of Excellence or other accredited health care facilities, preferably in remote or underserved populations to be determined with supervisor.

Skills:

The Healthcare Navigator will be a LSW, or LCSW, with experience working in a health care or hospital setting, preferably with the chronic illness population. Infusion Center experience a plus. RN, BSN may be considered for the right candidate. Experience 10 years , healthcare setting required. Bilingual (Spanish/English) a plus. Technical skills required: MS Teams, Zoom, MS Office Suite. Salesforce a plus, but not required.

This position is 30-40 hours per week, three days onsite. Conshohocken, PA. Some travel required. No phone calls, please.

Pay: $56,000.00 - $75,000.00 per year
