Responsibilities:
- Conduct comprehensive assessments of patients' health status
- Develop individualized care plans based on assessment findings
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patients' progress and adjust care plans accordingly
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with interdisciplinary healthcare teams to coordinate patient care
- Document all patient interactions, interventions, and outcomes accurately and timely
Requirements:
- Valid Registered Nurse (RN) license in the state of CA
- Minimum of 1 year of nursing experience in acute care or home health setting
- Proficient in patient assessment, medication administration, and medical imaging interpretation
- Knowledge of dementia care, workers compensation law, family planning, ophthalmology, and occupational health practices
- Excellent communication and interpersonal skills
- Strong critical thinking and problem-solving abilities
- Ability to work independently and as part of a team
