What are the responsibilities and job description for the Program Manager position at Goodwill Northern New England?

Job Summary:

1) Develop and direct Goodwill NNE’s community case management. Responsibilities include the direct supervision of Care Coordination and Casework Coordinators. Duties include:

  • Regular participation in proposals, marketing activities, service system initiatives and by meeting referral projections.
  • Meets licensing standards, CARF standards, quality indicator targets, corporate compliance standards, risk management targets, customer satisfaction goals, documentation standards, and program goals/objectives
  • Carries out social work duties: examples of duties include but are not limited to: individual engagement, intake, psychosocial histories, family contacts, assessments, planning, facilitation, coordination and advocacy

2) To manage the Care Coordination team ensuring that all service recipients’ needs are met.

  • Maintains relationships and coordinates services with interested parties inside and outside the Agency including; providers, family, guardians, government regulators, payors and the community.
  • Assures quality of designated program(s) consistent with Agency guidelines; including customer satisfaction, record keeping, incident reporting, medication administration, access and follow-up to medical care, participation in the development and implementation of service plans for the service recipient, and ensuring service coordination, licensing and accreditation.
  • Manages day to day operations of designated program(s) including personnel management.

3) Responsible for the advocacy, care coordination, case record maintenance, and communication with insurance companies of individuals with acquired brain injuries

  • Collects information about service recipients and their support system using interviews, case history, and observation. Confers with Care Coordination team, individually and in team meetings, to exchange, discuss, and evaluate the information for planning, modifying, and coordinating treatment programs.
  • Reviews service plans and performs follow-ups to determine the quantity, quality, and satisfaction of services provided.
  • Prepares service plans, progress summaries, meeting minutes, progress notes, and other documentation as needed. Presents information, written and verbal, to team members, service recipients, family members, payors, and other stakeholders in a manner that is understandable, timely, and helpful.
  • Verifies insurance authorizations and ensures clients do not exceed the authorized amount by tracking and obtaining follow-up authorizations
  • Participates in process improvement initiatives.

4) Represents the Agency in all interactions with other members of the Goodwill community and with the general public, supporting the agency Vision of creating sustainable communities that thrive through the fullest participation of their diverse

Minimum Qualifications:
  • Bachelors in Social Work or related field
  • Two years relevant human services experience
  • Licensed as or eligible for LSW, LMSW or LCSW in the State of Maine
  • Knowledge of community resources available to program participants including resources such as Department of Human Services, Office of Aging and Disability Services, Vocational Rehabilitation
  • Certified Brain Injury Specialist
  • Valid driver’s license permitting operation of a vehicle in the State of Maine with a satisfactory driving record
  • Criminal background check that meets Agency standards
