Urgently seeking a Care Coordinator with a well-established organization that works with individuals suffering from severe mental illnesses and disabilities.
This position is a Full Time Position.
Essential Duties of the Care Coordinator :
- Provide Assessments, Diagnosis, Service Planning, Case Management, and Crisis Services
- Conduct initial Assessments
- Conduct annual Assessments
- Update treatment plans
- Strength-based and trauma-informed approach
- Collaborate with team member, staff, and more
- Administrative work; complete paperwork, clinical records
Qualifications of the Care Coordinator :
- QMHP Certification or meet requirements to be credentialed as outlined by MHACBO
- Understand adhere to ethical standards of the governing license / certification board
- Master’s in Mental Health, Social Services, or similarly related Behavioral Health field
- Knowledge of and comfortable with Assessing and Diagnosing
- Comfortable working with patients that have mental illnesses and disabilities
- Communication
- Ability to perform physical requirements of the position to include : lifting, carrying up to 20 pounds, standing, sitting, kneeling, squatting, crouching, reaching, tolerate strong odors, repetitive motion, use of computers, local travel
