In this role, you will be responsible for performing OASIS assessments, creating personalized care plans, and coordinating services to meet the complex needs of individuals with chronic illnesses, disabilities, or developmental disabilities. You will oversee the provision of home nursing care, therapy, and support services, ensuring adherence to their established protocols. By applying your nursing expertise and following the nursing process, you will enhance the development, autonomy, independence, well-being, and social interaction of their patients.

Requirements:
- Minimum 1 year of experience with EMR systems (Preferred)
- Minimum 1 year of experience in home health (Required)
- Active RN License (Required)
- Wound-care experience (Preferred but not required)
