PURPOSE: The Chaplain is a core member of the interdisciplinary team and will provide for the spiritual needs of our patients, families, caregivers and staff including volunteers. The Chaplain will also provide pastoral care interventions, including pastoral counseling which is determined by a spiritual assessment and sensitivity to a variety of values, beliefs and religious orientations. This position seeks excellence in pastoral care, which requires an authentic demeanor, compassion and skill in dealing with the spiritual dynamics of illness, loss and death.
QUALIFICATIONS:
Master’s degree in Divinity or related field preferred
Advanced Training of at least one unit of clinical pastoral education preferred
Affiliation with College of Chaplains preferred
Two (2) years experience in health care preferred
Knowledge of the spiritual issues common during illness, death and bereavement
Endorsement by denomination preferred
Strong communication & interpersonal skills required
Excellent organizational skills
Ability to travel within the State of NJ
RESPONSIBILITIES (including, but not limited to):
This position does not have supervisory duties
Provide spiritual assessment, counseling and support to patients and families.
Participate in the preparation, evaluation and execution of the interdisciplinary care plan to assure that the aim of continuity and comprehensiveness of care are accomplished.
Provide spiritual care in a non-judgmental manner that honors the spiritual needs of the patient/family.
Work collaboratively as a member of the Interdisciplinary Team, IDT.
Facilitate linkage to community clergy as desired by patient/family.
Perform memorial and funeral services for clients and contracted facilities as required, but at least annually.
Complete initial Chaplain assessment in Brightree within 48 hours or within 5 days including day of admission per Medicare regulation. Provide a communication note to the IDT.
Document in Brightree 2 patient visits (face to face or telephone calls) which can include initial Chaplain and follow up assessments.
Initial bereavement assessment to be completed on paper at the time of the initial assessment.
Care plans are to be established at the time of initial assessment relevant to each patient.
Assessments and visits are to be completed in Brightree and synced by the end of occurring business day.
Prepare documentation for the clinical record per hospice, state and federal guidelines/standards. Documentation must be prepared and turned in according to these guidelines or disciplinary actions may be taken.
Demonstrate and apply knowledge of diverse religious, cultural/spiritual values and customs related to illness, death and mourning.
Organize and participate in hospice memorial services at least annually.
Participate in taking on-call responsibilities as necessary including weekends and evenings.
Participate in the Interdisciplinary Team meetings, staff meetings, and other meeting as required.
Demonstrate understanding of and adherence to professional Code of Ethics and related organizational policies.
Participate in professional liaisons with other clergy, pastoral and health care providers.
Maintain quality and productivity standards, participate in the measurement of these standards.
Participate in agency and community programs including supervision to enhance learning and performance.
Participate in the orientation of staff and volunteers as required.
Participate in providing support to staff and volunteers in coping with job-related stress.
Prepare and present presentations to families, caregivers and others.
Adhere to practice of confidentiality regarding patients, families, staff, and the organization.
Ensure that Grace Healthcare Services best practices are incorporated into all job activities/responsibilities.
Perform other duties as necessary.
