What are the responsibilities and job description for the Clinical Social Worker (MSW) position at Grand River Health?
BASIC FUNCTION: The Clinical Social Worker (MSW) is a part of the Grand River Hospital District Clinic's treatment team that identifies, triages, and provides mental health treatment to clients of all ages with behavioral health issues. This includes individual and family therapy as well as crisis assessment and intervention. The ideal candidate will work collaboratively with the other Mental Health Providers and Primary Care providers on staff and under the direction and supervision of Mental Health Supervisor.
RESPONSIBILITIES:
- Performs case management activities for clients referred to the mental health team including providing resources, triaging, scheduling and some clerical functions.
- Teaches clients, families and staff about care, prevention and treatment enhancement techniques.
- An additional area of focus for the MSW is the Inpatient Swing Bed program. As part of the multi-disciplinary Swing Bed team, performs rounding on Swing Bed patients to meet the psychosocial requirements of care and to provide mental health treatment and services as needed.
- Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems related to their biopsychosocial needs.
- Refers clients to other Mental Health staff as necessary for additional services and therapy.
- Participates as a member of multi-disciplinary team and may assist patients in treatment planning.
- May contribute to the formulation of discharge plans.
QUALIFICATIONS:
- Master’s Degree in Social Work.
PREFERRED QUALIFICATIONS:
- 1-2 years’ relevant experience in an integrated clinical setting preferred but not required.
SALARY INFORMATION :
$32.10 - $35.05 Dependent on Experience
$5,000 Bonus (If Eligible)
