Licensed Practical Nurse - Home Health
Duties:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments of patients' health status
- Administer medications and treatments as prescribed
- Monitor and evaluate patient progress
- Educate patients and their families on managing their conditions and promoting wellness
- Collaborate with other healthcare professionals to coordinate patient care
- Document patient assessments, interventions, and outcomes accurately and timely
Experience:
- Minimum LPN certification
- Experience in home health or a related field is preferred
- Proficient in standards of nursing practice and clinical procedures
- Knowledge of critical care, dementia care, and assisted living practices
- Experience in medication administration and management
- Proper documentation
- Strong understanding of infection control protocols
