Release of Information Representative

Responsibilities

The Release of Information Representative is primarily responsible for processing release of information requests including patient walk in requests, telephone requests, physician requests, mail requests, subpoenas and in house requests on a timely basis. They are to work closely with the Release of Information vendor to ensure all requests are processed in a timely manner. The incumbent is responsible for patient portal enrollments and related calls regarding patient portal. The incumbent is responsible for the Death Registry and the Birth Registry Process.

Essential Functions:

• Processes requests by mail, telephone, fax, email and/or from walk-ins for the release of information from the medical record for use by insurance companies, third party payers, lawyers, physicians and government agencies.
• Works in conjunction with the Release of Information vendor to ensure timely processing of all medical record requests.
• Interacts with Copy Service Representative to provide direction and continuity of workflow.
• Checks each request for properly executed authorization, signed by patient; goes to nursing units to obtain inpatient authorization as needed in accordance with regulations governing the release of information and following the Grand View Hospital policy for release of information.
• Exercises judgment against disclosure of confidential information to unauthorized users.
• Follows specified procedures for copying specified portions of medical records; compares signatures and call requestor with any discrepancies or clarifying questions.Maintains working knowledge of HIPAA regulatory guidelines regarding release of the information and consult with HIPAA Compliance Officer.
• Has a working knowledge of the electronic medical record (EMR) systems.
• Safeguards patient confidentiality at all times in accordance with hospital policy.
• Processes discharges to predetermined procedure in order to provide coverage for responding to patient care chart requests.
• Manages the electronic Death Registry process including the completion of Death certificates by medical staff.
• Manages the electronic Birth Registry process including the verification of Birth certificate information.
• Maintains department spreadsheets relating to the duties in the Release of Information Office.Represents the Health Information Management department of GVH in court cases, maintaining a professional demeanor at all times, in response to subpoenas requiring an appearance.
• Performs other duties as assigned for promotion of smooth general work flow including daily chart assembly/analysis.
• Obtain records from other hospitals and providers when inpatient physicians send an order for in-house patients.
• Offer training and education to GVH staff regarding release of information as needed.

Qualifications

Education and Experience:High School Diploma/GED And Over 1 year to 3 years general office experience. required

Licenses and Certifications:

HIPAA Privacy Certification , Within 6 months of hire, preferred
