Qualifications
- Registered Nurse or Licensed Practical Nurse with a current New Hampshire or compact state license.
- Experience in primary care and ambulatory health care setting preferred.
- BLS certification required.
- Experience with electronic health records a plus.
Job Duties
- Triaging patients
- Case management
- Immunization administration
- Patient education
- Monitoring of controlled medication refills
- Transition of care from hospitals to nurse visits
- Managing COVID-19 clinical workflows including scheduling testing, coordination treatment protocols, triaging COVID-19 diagnosed cases, managing vaccine administration, coordinating COVID-1 clinics and coordinating community COVID-19 education

Hours Per Week
Clinic hours are 8:00am-5:00pm, MWF; 8:00am-7:00pm, TTH. This position is still considered Full-Time though it is budgeted for 30 hours (or four days) per week. The schedule is flexible and to be discussed. Nights and Saturdays will be required on rotation in spring or summer of 2023.
