What are the responsibilities and job description for the Admission RN position at Guardian Angel Hospice?

Guardian Angel Hospice desires an Admissions RN who can combine working knowledge of Hospice Care regulatory standards with a proven track record in delivering and documenting quality care in the home. The ideal applicant will have the ability to present Guardian Angel Hospice services and philosophy to patients and families in a manner that results in the admission of appropriate patients.
The Admissions RN:
- Participates in the information gathering, analysis, education, and form completion aspects of the admissions process.
- Evaluates patients for hospice appropriateness.
- Obtains agreement for admission of referred patients who meet admission criteria.
- Maintains clinical and professional competence.
- Explains Guardian Angel Hospice services to patients and families.
- Utilizes interpersonal skills to help appropriately educate and address any questions the patient and family has concerning hospice care.
Job Requirements:
- Must have reliable transportation, valid driver’s license and be willing to travel within the company’s service area.
- Current Indiana RN licensure.
- Willingness to serve on the on-call rotation schedule.
- Ability to work in a field setting and to make sound nursing judgements.
- Ability to communicate effectively with patients and families, physicians, referral sources, and colleagues.
- Detail oriented and good time management skills
- Previous experience in Hospice, Home Health preferred
