AFC is a regulated program which facilitates patients receiving care from their loved ones in the comfort of their homes. Our agency organizes and manages care, including ensuring that caregivers are qualified and approved.

Please Note: We train nurses in AFC, if needed. No previous AFC experience is required.
POSITION SUMMARY:
Under the supervision of the Asst Program Director the Registered Nurse is responsible for the evaluation of the potential clients, caregivers and planning and continuous monitoring of existing AFC clients.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Conduct initial assessments and evaluations of the client's appropriateness for AFC, with ongoing monitoring as needed.
- Complete the MDS-HC client assessment for prior authorizations, initial, annual and other nursing assessments.
- Conduct monthly or bi-monthly client visits per regulations for Level I or Level II or as needed, such as wellness checks.
- Develop and review on an ongoing basis each client's AFC plan of care.
- Train, evaluate, and supervise AFC caregivers in conjunction with the case manager in conjunction with the physician summary.
- Complete nursing progress notes for each on-site visit or encounter and upon significant change.
- Monitor each client's health status and document findings in the client's medical record for each on-site visit or encounter, or more often as the client's condition warrants.
- Coordinate the implementation of physician's orders with the AFC caregiver, and AFC provider personnel.
- Submit a semiannual health-status report to the client's primary care physician as applicable.
- Plan and implement discharges from the AFC program as applicable.
- Conduct an orientation for each AFC caregiver before the AFC caregiver begins personal care.
- Provide ongoing training/coaching to AFC caregivers on health, aging, nutrition, and other relevant topics.
EDUCATION & QUALIFICATION REQUIREMENTS:
1. Graduate of an accredited School of Nursing, College or University.
2. Currently licensed and registered to practice with the Massachusetts Board of Nursing.
3. Experience in the direct care of elderly or disabled adults is preferred.
4. Current CPR certification
5. Driver's License and Reliable Vehicle
