Position Hours:

Full-time: Monday-Friday 8:00a.m.-4:30p.m.

Position Qualifications & Requirements:

• Valid Louisiana RN license required
• Ability to meet physical demands of the job
• Positive attitude
• Dedication to quality patient care
• Strong communication and critical thinking skills
• Willingness to develop working knowledge of Federal and State Regulations/Conditions of Participation as they relate to the referral and admission process for Hospice.
• Willingness to develop working knowledge of Local Coverage Determinations (LCDs).
• Ability to work well with physicians and facilities.
• Basic computer skills.
• Punctual, reliable and available during routine scheduled hours. Occasionally duties may include hours outside of these.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Willingness to participate in appropriate education as requested by immediate supervisor.
• Contribute to team effort by accomplishing related results as needed
• Prepare clinical notes
• Other duties as assigned

Responsibilities:

• Oversee and provide patient care visits.
• Develop patient plan of care.
• Coordinate with patient's family, physician and caregivers.
• Completes and submits timely clinical documentation that is accurate, comprehensive, and reflective of the appropriate terminal diagnosis.
• Communicates with other team members and participates in the IDT process.
• Develops the initial and ongoing plan of care with collaboration from other team members.
• Participates in the QAPI program and assists with chart reviews as assigned.
• Promotes Guardian Hospice Care in the community.
• Supports the Guardian Hospice Care Mission and Values.
• Completes other projects and duties as assigned.
