Position Hours:
Full-time: Monday-Friday 8:00a.m.-4:30p.m.
Position Qualifications & Requirements:
- Valid Louisiana RN license required
- Ability to meet physical demands of the job
- Positive attitude
- Dedication to quality patient care
- Strong communication and critical thinking skills
- Willingness to develop working knowledge of Federal and State Regulations/Conditions of Participation as they relate to the referral and admission process for Hospice.
- Willingness to develop working knowledge of Local Coverage Determinations (LCDs).
- Ability to work well with physicians and facilities.
- Basic computer skills.
- Punctual, reliable and available during routine scheduled hours. Occasionally duties may include hours outside of these.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Willingness to participate in appropriate education as requested by immediate supervisor.
- Contribute to team effort by accomplishing related results as needed
- Prepare clinical notes
- Other duties as assigned
Responsibilities:
- Oversee and provide patient care visits.
- Develop patient plan of care.
- Coordinate with patient's family, physician and caregivers.
- Completes and submits timely clinical documentation that is accurate, comprehensive, and reflective of the appropriate terminal diagnosis.
- Communicates with other team members and participates in the IDT process.
- Develops the initial and ongoing plan of care with collaboration from other team members.
- Participates in the QAPI program and assists with chart reviews as assigned.
- Promotes Guardian Hospice Care in the community.
- Supports the Guardian Hospice Care Mission and Values.
- Completes other projects and duties as assigned.
