Hiring Range:             $46,000 -$50,000

 

Purpose

This position is responsible for providing mobile case management services to individuals experiencing homelessness or those that have a history of homelessness.  This team provides flexible services to support people in establishing and maintaining housing to improve their quality of life. The purpose of the team is to creatively engage individuals in a deeper level of service, connect them to needed/desired resources and assist them with finding permanent housing situations that best fit their needs. The end result is assisting individuals to lead healthier, more stable lives.  This position will work with individuals who are diagnosed with serious and persistent mental illness, substance use disorders, co-occurring medical conditions and those who are experiencing chronic homelessness 

 

Key Responsibilities

  • Engage with participants to build rapport and trust.
  • Provide flexible, individualized support so individuals can lead self-directed lives. 
  • Assess needs, identify goals, and plan services so that individuals’ needs are met, and goals are enhanced.
  • Partnering with the individual to find, establish and maintain permanent housing situations that best suit the individual’s needs with the help of scattered site housing vouchers.
  • Connect participants to additional needed services and resources.
  • Assess readiness for change and follow Housing First, Person Centered and Harm Reduction approaches.
  • Communicate consistent information and aid the team to ensure continuity of service for the individuals served.
  • Facilitate communication among others involved, to provide coordination and define service direction to attain individuals’ goals.       
  • Provide crisis assessment and intervention so individuals receive the assistance they need to be safe in their home and community. 
  • Ensure services are documented in individual records so information is readily available, results of services can be determined, and record-keeping standards are met.       
  • Promote an inclusive, diverse, culturally competent, and respectful workplace.

Education and Experience 

REQUIRED:

Bachelor’s Degree in one of the behavioral sciences (social work, psychology or human services) or a related field from an accredited college or university AND has 2,000 hours (1 year) of experience in the delivery of service to persons with mental illness.

OR

Qualified as a mental health practitioner within the state of Minnesota by one of the following:  

  • At least 30 semester/45 quarter credits AND 2000 hours of supervised experience in the provision of mental health services. 
  • 4,000 hours of supervised experience in the provision of mental health services. 

 

NICE TO HAVE EXPERIENCE:

  • Experience providing Targeted Case Management services.
  • Experience providing homeless outreach services and/or serving people experiencing homelessness.
  • Knowledge of resources in Ramsey and/or Hennepin County.

 

ADDTIONAL REQUIREMENTS:           

  • A valid Driver’s License, reliable vehicle, satisfactory driving record, and auto insurance are required.
  • Have a working knowledge of person-centered planning, harm reduction, and the needs of individuals with SPMI/CD.    
  • Must be able to perform job duties in the community, be able to negotiate stairways and have the ability to lift and carry items up to or exceeding 40pounds.
  • Must have the ability to pass a DHS background study.
  • Must not be actively receiving services from Guild and must not have received services within the last two years.
