Job Summary:

The Stroke Program Manager is responsible for the daily oversight and management of the Comprehensive Stroke Center (CSC) across the care continuum along with the Stroke Medical Director. This includes stroke program certification, patient, and staff education, as well as oversight of interdisciplinary/interprofessional collaboration for optimal patient care and safety. The Stroke Program Manager is a recognized expert in their field. Adheres to all best practice processes and guidelines as noted by national literature and ensures implementation within the center, the hospital, and the region. The Stroke Program Manager works collaboratively with all levels within the organization, across the HMH system, and with community partners. They work under the direct supervision and direction of the Stroke Medical Director and indirect reporting to the Director of Network Neuroscience Quality.

Essential Job Functions:

1. Direct the development and revision of policies, procedures, standards, & expectations for the management of Stroke patients. Ensure ongoing compliance with best practices.

2. Coordinates the care of stroke patients using the HUB and SPOKE model across the region.

3. Co-Chair Stroke Administrative Committee as well as Stroke Collaborative and facilitate regular meetings. Participates in committee meetings requiring stroke representation throughout the hospital, region, and HMH Network.

4. Responsible for ensuring interpretation, execution and compliance with standards for stroke center certification.

5. Identifies and maintains concurrent list of inhouse stroke patients and monitors clinical progress

6. Evaluates customer satisfaction and implements processes to establish improvements.

7. Leads preparation and attainment of program certification, often in collaboration with regulatory, quality and patient safety, and nursing leadership

8. Actively collaborate with network neuroscience quality leadership to ensure quality data systems, performance improvement process and data analysis are in place

9. Collaborates with the marketing department to provide for consumer brochures for the program. Provides community organizations with information about services offered to patients. Works with outreach team to promote service and community awareness

10. Actively participates in local, regional, and national advisory groups to work to advance the science of care for the stroke patient. Represents hospitals at local, regional, and national meetings demonstrating and articulating the model for clinical excellence.

11. Reviews current medical and nursing research literature and demonstrates application of research to clinical practice. Participates in patient centered research studies and works with IRB where appropriate. Maintains CITI training to act as Sub-investigator when necessary

12. Responding to and supporting provision of excellent emergent acute stroke care (Inhouse and ED Code Stroke, as needed).

13. Provides concurrent and retrospective review and analysis of the care of stroke patients at the patient level, program level and institutional level

14. Coordinates clinical care workflows in collaboration with the Stroke Medical Director and with the Stroke team including physician providers, APPs, social work, PT/OT/Speech, etc.

15. Functions as expert resource for transfer center, EMS, and critical care transport teams for safe delivery of stroke patients. Works in conjunction with Stroke Performance Improvement Advisor and Stroke APP Team Leader

16. Responsible for developing and ensuring appropriate use of patient education materials.

17. Addresses and assists with coverage for programmatic gaps at primary stroke center spoke sites, as needed.

18. Responsible for developing, coordinating, and ensuring delivery of regular stroke education to nurses, physicians, clinicians, patients, families, Emergency Medical Services (EMS), communities, referring hospitals

19. Analyzes population data and trends within and outside of institutions to project need for education, services, and community interventions. Educates prehospital providers, community medical providers, and general public about stroke recognition, treatment

20. Utilizes the stroke registry and other data platforms (EA&I, Vizient, Looker, IQVIA) to develop robust dashboard to monitor clinical effectiveness and patient outcomes.

21. Able to analyze quality performance data, perform gap analysis that explore and clarify areas for program improvement, and construct and lead presentations for stroke quality conferences

22. Collaborates with the quality management department & stroke registry staff to ensure timely and accurate data abstraction/entry occurs

23. Adheres to HMH Organizational competencies and standards of behavior

24. Other duties and/or projects as assigned.

Education, Knowledge, Skills, and Abilities Required:

1. Master’s Degree in Nursing.

2. A minimum of 3 years of clinical hospital experience, preferably in neurology, intensive care, or emergency care.

3. Strong presentation and facilitation skills.

4. Excellent written and verbal communication skills.

5. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills, and Abilities Preferred:

1. Not applicable.

Licenses and Certifications Required:

1. NJ State Professional Registered Nurse License.

2. National neuro certification within one year of hire (SCRN, CNRN, NVRN)

Licenses and Certifications Preferred:

1. Certified Neuroscience Registered Nurse.

2. Advanced Cardiac Life Support Certification.

3. AHA Basic Health Care Life Support HCP Certification.

Job Type: Full-time

Pay: $120,000.00 - $165,000.00 per year
