Demonstrate understanding of the LCD Guidelines (with training) for hospice eligibility and key regulations for all levels of hospice care.
Conducts an initial and comprehensive patient assessment, inclusive of physical, psycho-social, and spiritual entities. Gathers accurate information from all pertinent sources and completed patient assessment to create an initial Care Plan within 48 hours of admission.
Demonstrates use of the interdisciplinary approach in developing an individualized plan of care that optimizes patient and family goals and reduces suffering. Reviews and revises as necessary.
Provides education for patient, family and interdisciplinary (IDT) team members about disease trajectory, common treatments and complications, social and psychological issues associated with life-limiting illness.
Demonstrates pain and symptom management through assessment skills, understanding of use of opioids, non-opioids analgesics, adjunctive agents, and other pharmacological and non-pharmacological approaches.
Recognizes signs and symptoms of impending death and appropriately cares for the imminently dying patient and his/her family members.
Demonstrates interpersonal and communication skills that result in effective leadership, relationship-building, information exchange, emotional support, shared decision-making, and teaming with patients, families, and professional associates.
Leads, organizes, and collaborates effectively in IDT, family, and patient-focus meetings to include care coordination meetings with facility staff.
Recognizes age, gender, and culture-appropriate concepts, practices, and language.
Demonstrates skill in computer technology, Matrix EMR (with training) as a documentation tool, resource, and medical record.
Promotes, observes, and teaches patient safety. Reports signs and symptoms of abuse, neglect, exploitation, and/or unsafe environments to appropriate individual(s) per company policy.
Provides and promotes infection control program for surveillance, identification, prevention, control, and investigation.
Supervises Hospice Aide services no less than every 14 days, and on site-direct visit annually or as deemed necessary.
