Case Manager
Description of Duties and Responsibilities
· Assessment of the individual needs of those at-risk or experiencing homelessness
· Process referrals to appropriate service providers
· Provide community services to participants who are chronically homeless with a mental health diagnosis and/or co-occurring disorder
· Development and review of Service Plans with individuals served
· Advocacy as needed and deemed appropriate to insure quality service delivery
· Maintains client confidentiality and adheres to the clinical ethical standards
· Coordination, cooperation and communication with agencies serving participants with mental illness and co-occurring disorders
· Provide crisis intervention services focused on enhancing the clients’ ability to independently problem solve, use effective coping skills, and manage and self-coordinate own care
Qualifications for Position
Bachelors of Social Work or social service related field required; At least one year experience providing case management services in a non-profit, for profit, government, or public service environment preferred: Homeless services, case management, mental health, and permanent supportive housing experience preferred
Supervisory Relationships: There are no supervisory responsibilities for this position.
Skills and Knowledge Required
Ability to work with people of all backgrounds and sensitivity to the problems of the homeless; Knowledge, experience and understanding of mental illness and substance abuse; General knowledge of community services and referral process; Ability to interact constructively with diverse community interests
Personal Qualities
A high level of understanding for individuals who present for services with multiple case management and mental health needs; Ability to work independently and within a team environment and exercise mature judgment; Strong communication skills
