Job Shift: Shift 1
Shift Details: Monday-Friday including rotating weekends and holidays

Job Description:  LPN Hospice Homecare Visit Nurse to provide hospice care.

Ensure compassionate delivery of care to patients in their final journey.  Will work with interdisciplinary team of social workers and pastoral counselors to provide families and patients with understanding and support during this process.

Qualifications

Licensed Practical Nurse with a license to practice in the State of Connecticut

One year of Home Care experience preferred, one year of nursing experience required.
