JOB SUMMARY:

Assists the nursing staff by performing routine tasks related to meeting the daily physical and psychological needs of patients under the direct supervision of the RN or LPN. Assists with tasks necessary for the general management and organization of the nursing unit and Extended Care Facility, being flexible in meeting patient or unit needs.

ESSENTIAL JOB DUTIES & RESPONSIBILITIES

Performs responsibilities related to the provision of patient care, within the scope of practice of a NA.

  • Observes patients' conditions and reactions.
  • Notifies RN/LPN/Charge Nurse of patient's condition and any change to patient/resident condition, treatments, and significant incidents.
  • Measures and record food and liquid intake and output.
  • Takes vital signs.
  • Contributes to, monitors and implements patients plan of care.
  • Responds to call lights and patient needs in a timely fashion.
  • Recognizes changes in patient condition and notifies charge nurse in timely manner.
  • Serves as patient advocate.
  • Assists with direct clinical care as assigned/scheduled under the supervision of a RN/LPN and charge nurse.

Observes the patient at regular intervals and communicates information the Charge Nurse should know.

  • Reports to the Charge Nurse any observations and/or symptoms indicative of patients changes and adverse reactions to treatments and/or response of complaints in a prompt manner.
  • Reports care for assigned patients.

Participates as an effective member of the Care Team.

  • Responds in a timely and effective manner to patient/family concerns and provides immediate interventions when possible.
  • Provides written communication of concerns to necessary departments for appropriate follow up.
  • Adapts to changes in workload and completes assignments while maintaining quality care.
  • Initiates and performs duties which need to be completed although not directly assigned.
  • Promotes teamwork through effective interpersonal relationships with rest of staff.
  • Demonstrates initiative and flexibility in provision of care.

Maintains supplies and equipment.

  • Maintains knowledge of location for needed equipment and supplies.
  • Restocks supplies as needed.
  • Reports equipment needing repairs.
  • Cleans and stores equipment when no longer needed.

Performs and maintains documentation.

  • Documents accurate and ongoing assessments of patient status that reflect nursing interventions, patient responses, patient teaching and status of outcomes at discharge.
  • Maintains documentation of performance measures and quality improvement efforts.
  • Completes shift end check prior to leaving duty.

Communicates effectively.

  • Communicates and collaborates effectively with other health team members regarding patient condition, nursing plan of care, patient satisfaction needs and recommendations for meeting identified outcomes.
  • Follows approved shift report format when communicating patient information.
  • Identifies and recognizes abnormal symptoms/changes in patient condition, established priorities, and takes appropriate action. Appropriately reports condition changes to medical provider.

Promotes and ensures patient safety in performance of all responsibilities.

  • Administers medications, treatments and procedures in a timely and safe manner according to health care provider orders and nursing policy, including appropriate performance of standard precautions, using two patient identifiers, and performing patient risk assessments every shift.
  • Demonstrates proficient technical/clinical skills and operational knowledge of equipment on unit.
  • Utilizes proper body mechanics, transfer/lifting techniques to minimize fall risk to patient and injury to self.
  • Implements infectious disease controls as appropriate including isolation procedures, personal protective equipment (PPE), proper use of hand washing and gloves.
  • Follows proper procedures for wasting narcotics and disposal of sharps and hazardous materials.

Performs other duties as assigned.


QUALIFICATIONS:

  • High school diploma or equivalent
  • Current Colorado State Certified Nurse Aide (C.N.A.) preferred
  • Basic Life Support (BLS)
  • Previous Nurse Aide experience preferred

GENERAL REQUIREMENTS:

  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of organization.
  • Ability to add and subtract two-digit numbers and to multiply and divide with 10's and 100's.
  • Ability to perform these operations using units of American money and weight measurement, volume, and distance.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
  • Ability to deal with problems involving several concrete variables in standardized situations.
