Job Description:
The Community Liaison represents HCA hospitals throughout Miami-Dade County by providing customer service, communication and maintaining excellent relationships with judicial/legal representatives, mental health providers, healthcare professionals, law enforcement, healthcare facilities and other community organizations working with mental health consumers. The position incumbent facilitates communication between hospital departments/physicians and outside parties in need of hospitals services. This position functions as a patient and referral source navigator to facilitate a positive patient-centered experience for Behavioral Health service inquiries, and to assist with continuity of care for patients being discharged from hospital. The Community Liaison maintains a strong hospital presence in the community by attending relevant court hearings, community boards/meetings, mental health/substance abuse task forces, and community service organizations.
Qualifications
- High School Diploma or equivalent preferred
- 3-5 years experience
- English required.
- Bilingual in Spanish preferred
