HCA Houston Healthcare West – Houston, TX
Position - DSREP
HCA Houston Healthcare West is a full-service 266 bed hospital that provides a range of health services. It is recognized as a Certified Primary Stroke Center by the Joint Commission and, under an affiliation agreement with The Woman’s Hospital of Texas, brings to its community The Woman’s Hospital of Texas at West Houston, a three-story cardiac and critical care tower. The Sugar Land Cancer Center and the Sugar Land Diagnostic Center, departments with HCA Houston Healthcare West, are located on the Southwest Freeway at the Williams Trace and Sugar Lakes exit.
HCA Houston Healthcare West is affiliated with HCA Houston Healthcare, part of the HCA Healthcare Gulf Coast Division. The division is a comprehensive network of hospitals, outpatient surgery centers, emergency centers and diagnostic imaging facilities in greater Houston, Corpus Christi and South Texas. Facilities include: 17 hospitals, nine ambulatory care centers, 13 off-campus emergency centers, and a regional transfer center.
What you will do in this role:
The ER case manager will coordinate the care and service delivery of selected patient populations across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes. Physical and psychological factors including the identification of cases with potential for high-risk complications and acting as an advocate for an individual’s health care needs are essential elements of the position. Timely and appropriate discharges will be accomplished through assessment of the patient’s post discharge needs and the provision of services necessary to meet those needs. The ER case manager must be knowledgeable in community resources and alternate care facilities. Interacts with physicians, staff, patient/family and outside agencies. Serves to facilitate admission/discharge process: Initiates conflict resolution and refers these issues to the appropriate manager. Collaborates with Director of Emergency Department, Resource Management and Administration to meet facility goals with regard to appropriate utilization of these resources.What qualification

What you will need:
- RN or LVN with at least 5 years clinical experience in an acute care environment. Utilization review or case management background, one year, preferred. Knowledge of federal and state requirements. Working knowledge of community and governmental resources as well as knowledge of private payor regulations desirable.
- Current licensure as an RN/LVN by Texas board of Nursing; BSN, preferred. Case Manager certification by case management accreditation board, preferred.
