Job Summary and Qualifications

Case Manager (Registered Nurse)

Department: Inpatient Rehab

SCHEDULE: PRN / Per Diem / As Needed

The Case Manager will help facilitate discharge needs of patients with emphasis on culture, financial considerations and family dynamics. The Case Manager will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. The Case Manager will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.

ESSENTIAL FUNCTIONS (including, but not limited to):

  • Assists patient/families with psycho-social, cultural and financial considerations that may impede recovery.
  • Formulates plan for post-hospital care.
  • Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients appropriate to the ages of patients.
  • Facilitate weekly team meetings and family conferences as needed.
  • Completes psycho-social assessment upon admission.
  • Performs insurance updates, as requested.
  • Assists with post discharge phone calls and ensures post discharge arrangements are completed/delivered.

What qualifications you will need:

  • Associates Degree in Nursing or Diploma in Nursing required; BSN preferred
  • Florida Registered Nurse (RN) license and/or approved multi-state RN license required
  • Certification in Case Management, Nursing, or Utilization Review, preferred
  • Minimum of 3 years of nursing experience required
  • Case Management experience preferred
