What are the responsibilities and job description for the Certified HHAs or PCAs - Oswego County position at HCR Home Care?
Position Summary:
Provide personal care and supportive services to individuals in the community in the privacy of their home.
Qualifications:
HHA/PCA 2 certification in good standing according to NYS regulations. Must be mature and able to deal effectively with the job and its demands. Must be respectful of the patient’s rights and treat a client and his/her belongings with dignity and respect. Have a sympathetic attitude towards the care of the sick and disabled.
Job Description:
· Provide health related services as outlined in the plan of care authorized by a physician and established by a registered nurse or licensed therapist under the direction of the Aide Matrix established by the NYS Department of Health.
· Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.
· Immediately report changes in client’s condition to supervising professional.
· Complete and submit necessary documentation including charting tasks and observations on android device.
· Attend ongoing in-service programs in a timely manner to maintain certification.
· All other duties as assigned.
Requirements:
· Ensure the safety of the patient, patient’s family and/or the employee.
· Maintain HIPPA.
· Provide one’s own transportation to and from assignments, and arrive promptly to all assigned HCR work locations.
· Commit to be available for work and accept assignments when offered.
· Be able to speak and write in English in order to follow the plan of care, communicate effectively with patient and interdisciplinary team.
The physical demands described here must be met by an employee to successfully perform the essential functions of this job, and to ensure patient safety at all times in the household. While performing the duties of this job, the employee is required to visually recognize objects within 20 feet, have the ability to speak and hear.

· Sufficient hand strength and dexterity to carry out care plan duties

Additional Information:
· You are expected to work your agreed upon availability for 6 months at a time. All changes to your availability require manager’s approval.
· 24 hour cases require an aide to be in the home at all times; if you agree to work a shift on a 24 hour case you are expected to remain in the home until the aide covering the following shift arrives. If you are not able to commit to this requirement please do not accept a 24 hour case assignment.
· Working conditions may expose the home health aide and / or personal care aide to smoke, pets and other allergens.
· Potentially hazardous materials such as those found in cleaning agents and other chemicals may be stored in a client’s home.
· HCR offers a paid CNA to HHA transition class for new hires that currently hold a valid NYS CNA certificate with the expectation that graduates will remain employed with HCR Home Care for a minimum of one year.
