We've got a great opportunity for a Registered Nurse to join us at our new integrated family care clinic, Health Solutions Family Medicine at Perry Hall, located at 1591 Taos Rd. in Pueblo, CO.
What You’ll Be Doing
As a RN with outpatient services, you would be providing direct care and care coordination services to patients in an outpatient environment. You will be conducting assessments, managing cases, and providing supportive therapy with utmost kindness and care. Your attention to detail and problem-solving skills will be important to your responsibilities related to evaluating and managing medications. Furthermore, you'd have the distinct opportunity to work closely with a multidisciplinary team of Health Solutions professionals to serve individuals and groups.
Specifically, the Registered Nurse
Administrative Duties
- Check patients in and out.
- Keeps accurate, complete, and up-to-date records with 100% compliance with set standards.
- Maintains respect for privacy by adhering to confidentiality in all phases of work.
- Adheres to the spirit of the mission statement while performing assigned duties.
- Maintains competency and proficiency with the electronic health record.
- Conducts intake and nursing assessments.
- Provides case management and supportive therapy to a caseload of physician/behavioral health patients.
- Assists clients with organizing and managing medication boxes.
- Administers medication by injection as ordered to clients in the clinic.
- Receives and records provider orders.
- Maintains contact with pharmacies to assure accurate and timely medication orders are met.
- Facilitates lab work, reviews results, and brings results to the attention of providers as needed.
- Administers vaccine and medication injections, (IM and SC)
- Performs venipuncture.
- Witness’ and collect urine samples.
- Provides medication education for clients.
- Participates as a team member in meetings.
- Keeps accurate and timely records with 100 percent compliance with our standards.
- Conducts AIMS, Daily Living Assessment, and depression screening for all clients.
- Coordinates care with community agencies, specialists, assisted living, and long-term care facilities.
- Supports and includes family members and others involved in the care of clients.
- Supports other departments and units as needed; and
- Performs other job-related duties as assigned by the program Supervisor, Director, and Vice President of Medical Services.
- Perform coverage for care coordination RN/MA.
- Will be required to cross train in other duties as allowable by licensure.
- Performs other job-related duties as assigned by a supervisor/director.
Physical Requirements
- Occasionally moves equipment and supplies as well as patients with mobility limitations.
- Frequently uses medical equipment such as venipuncture needles and blood pressure devices.
- Frequently stays stationary for prolonged periods.
- Frequently walks for prolonged periods.
- Frequently exposed to communicable diseases and bodily fluids.
- Frequently exposed to stressful or emergent situations.
- Frequently operates a computer and other office equipment to record health data.
- Consistently communicate with patients.

Qualifications
What we’re looking for—The Must-Haves
- Current license as a registered nurse in Colorado or any state participating in the Enhanced Nurse Licensure Compact
- Associate or bachelor's degree in nursing from an accredited academic institution
- Experience with and knowledge of the effective use of psychiatric medicine
- Experience working successfully with a multi-disciplinary team, especially in an integrated primary care setting.
- Ability to work in a fast-paced primary integrated healthcare practice.
- Experience providing clinical services to patients of all ages.
- Experience and comfort working with a wide range of behavioral health conditions and presentations to include substance use disorders.
- Experience working with severe and persistently mentally ill adults, and experience with various behavioral health diagnoses including substance use.
- Good communication skills, both verbal and written.
- Exceptional time management skills.
- Flexibility and ability to easily adapt to change.
- Well-developed documentation skills.
- Computer proficiency.
- Valid Colorado driver’s license, and access to reliable transportation during work hours.
- CPR/First Aid
