The Activities Coordinator is responsible for actively participating in all care coordination activities including Assessments and Care Planning.
The Activities Coordinator is responsible for utilizing variety of techniques including but not limited to arts and crafts, animals, sports, games, dance and movement, drama, music, and community outings.
The activity program shall also provide a planned schedule of social and other purposeful independent or group activities.
This position is responsible for developing and implementing therapeutic activity program, to improve or maintain physical, cognitive, social, emotional and spiritual functioning in order to facilitate full participation in life.
Activities Coordinator may use remote technology depending on the participants’ comfort level, electronic devices, and interests.
Corporate Setting the Activities Coordinator is responsible for developing and implementing therapeutic activity programs.
This position will also assist the Recreational Manager in the development and execution of recreational activities projects for process improvement and enhancement of participant experience.
Responsibilities :
1. Developing groups and programs in consideration of the needs, abilities, and interests of the participants. The provision of programs shall offer a variety of formats including small group, one on one, and large group, either in person, telephonically, or via video technology.
2. Assess, plan, prepare and carry out individually designed activity-based interventions to help maintain or improve physical, cognitive, social, emotional and spiritual functioning in order to facilitate full participation in life.
3. Assess, plan, prepare, and carry out video activities to all GrandPad users through Zoom as scheduled in the master calendar of GrandPad activities.
4. Active participation and contribution in the IDT including the coordination and development of an individualized participant plan of care, services and other activities.
Actively evaluates the participation by participants, and communicates, collaborates and updates other IDT members to assist with coordination of care as needed.
5. Plan, develop, implement, and supervise the activity program to encourage participant involvement and receive assistance from the Personal Care Attendant as needed.
6. Monitor and observe participants while conducting the Activities project and report any changes in a participant to the Interdisciplinary Team (IDT) and / or provider as appropriate.
7. Initiating, planning and posting of the monthly calendar in conjunction with the Care Center Supervisor as well as communicating effectively with the Personal Care Attendant to initiate daily activities programs.
The calendar shall include all monthly activities such as crafts, special events, religious services, and outings.
8. Provide guidance and direct oversight to contracted vendors / volunteers while performing or assisting at the Center to ensure they follow all policies and protocols.
9. Assists with transporting and / or escorting participants to, from and within the center, and on community activities (i.e. outings).
10. Plans, coordinates with transportation and other staff, and participates in community outings.
11. Monthly management of ordering / shopping for supplies, conducting inventory duties, accurate tracking of invoices / receipts and maintaining expenses within allowed budget.
12. Attend meeting and training routinely and as needed.
13. Conducting surveys to measure participant satisfaction.
14. Collaborate with the Recreational Manager in the design, implementation, and monitoring of recreational programs.
15. Adhere to project target schedule and deadlines.
16. Provide training, supervision, and evaluation of activity personnel.
17. Evaluate potential problems and technical difficulties with established recreational programs and develop solutions.
18. Perform all other related duties as assigned.
1. High school diploma required; current college coursework preferred.
2. Minimum of 1 year of experience working with the elderly in a social or recreational program preferred.
3. Completion of the Activity Leader (previously Activity Director ) course designed specifically for this position and approved by the State of California required prior or within 30 days of starting new role.
4. BLS / CPR training required.
5. Bilingual : English / Spanish / Mandarin / Cantonese depending on location preferred.

Physical Requirements
1. Able to stand, sit, talk, stoop, lift (up to 15lbs.) for carrying out routine duties of position.
2. Able to stand for prolonged periods in leading and performing activities
3. May be required to travel to sites / program / participants’ homes and special functions as needed.
