Job Summary
The Weekend On-Call RN organizes and directs all patient care and communications during their after-hours weekend shifts. The Weekend On-Call RN provides direct patient care and visits to patient's homes when needed, and provides care with a prompt and accurate telephone consultation. The weekend on call shifts are Saturday and Sunday from 8:00 am to 4:30 pm. Per-diem employees are asked to provide availability for three shifts per month.
Essential Job Functions/Responsibilities:
- Assures the highest quality of care is provided and documented in accordance with the plan of care.
- Provides support and education to patients and caregivers after hours.
- Responds promptly to urgent needs or significant changes in patient condition.
- The on-call registered nurse will visit the patient or family, when necessary, and offer a visit to patient and caregiver.
- The on-call registered nurse will arrange for any other hospice services, as needed.
- Reports will be given a) daily on on-call activities and b) to RN Case Managers as needed for individual reports.
Position Qualifications:
- Registered nurse with current licensure to practice professional nursing in the state..
- Maintains a current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Minimum of two years experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
- Self-directed and able to work with minimal supervision.
- Demonstrates excellent observation, problem-solving, verbal and written communications; nursing skills per competency checklist.
Physical Requirements:
- May be required to climb 15 stairs, if patient resides in a building where there are no elevators, while carrying a nursing bag containing a computer and nursing supplies and weighing up to 20 pounds.
- Must be able to stand in a patient’s home for at least 40 minutes at a time without sitting in the event adequate seating is not available.
- Must be able to bend over a bed and roll/hold the patient for several minutes at a time in order to assist others or perform duties on their own.
- Must be able to lift, position and/or transfer a patient in a bed, bed-to-chair or chair-to-bed, or assist with lift, position and/or transfer.
- Must be able to lift up to 20 pounds of supplies and equipment needed in all situations.
- Must be able to balance self and transfer a patient successfully with a gait belt and/or able to lower a patient to the floor “purposely” and safely without injury to the patient or self if the patient is unable to maintain weight bearing.
- Must be able to squat to the floor or get on knees on floor to perform wound care and manage dressing changes, such as on feet and legs, of patients, as well as performing catheter placement and catheter care, then return to standing position.
- Must have visual acuity and hearing to perform required nursing skills.
Benefits for Per-Diem Positions:
- Shifts are paid at a piece rate of $250 per shift.
- Sick time accrues at the rate of 1 hour for every 40 hours worked.
- Miles driven for work are reimbursed at 62.5 cents per mile.
- Ability to enroll in 401(k) and begin contributing at time of hire. Heartlinks will match dollar for dollar up to 5% of earnings after one year of employment and 1080 hours worked.
