Job Summary:
The Help at Home Population Health Manager is responsible for improving group health outcomes through targeted programs.
Responsibilities:
- Work with communities to analyze health data and identify policies and programs that can improve outcomes and address inequities
- Collaborates with the Care Coordination team to develop clinical programs for specific populations
- Design and analyze performance and outcome metrics for clinical programs
- Lead and sustain program integrity and efficacy
- Analyze population health and utilization metrics to determine areas of focus for improvement
- Prepare and present population health program performance metrics for internal and external stakeholders
- Facilitate the utility of data integration from external sources to identify opportunities and drive population health outcomes
- Create meaningful and insightful reports from data repositories
- Collaborate with Information Technology and Data and Analytics to optimize population health performance
- Works collaboratively with stakeholders to enhance clinical programs, clinical systems, and Care Coordination team performance
- Maintains subject matter expertise in clinical practice, quality metrics, and health industry trends
- Responsible for additional population health management tasks for evolving business initiatives
Qualifications:
- Education: Bachelor's Degree in Public Health or related fields
- 5 years of experience working in health plans, health systems, or provider organizations, managing population health
- Demonstrated ability to build rapport, collaborate, and develop effective working relationships with all levels of staff and key stakeholders
- Demonstrated clinical and business acumen with specific understanding of value-based care
Knowledge and Skills:
- Very strong understanding of clinical programs and pop health initiatives including quality metrics such as HEDIS and HCBS
- Strong statistical knowledge
- Project management skills and ability to manage multiple programs at once
- Ability to create a successful program with a clear mission, timeline, and benchmarks to measure progress
- Strong critical thinking and problem-solving skills
- Ability to work independently and as part of a broader Care Coordination team
- Excellent interpersonal, written, listening, and verbal communication skills
- Organized, flexible and able to multitask
- Ability to manage ambiguity and work within a rapidly changing environment
- Ability to anticipate requests and needs in advance and plan accordingly
- Strong virtual presentation skills
- Proficient in MS Office
