Description
The Patient Resources Coordinator works closely with administration, medical providers, primary care teams, and social services agencies to provide short-term care coordination and connection to resources to support clients in improving their health and general well-being. Implements and oversees off-site/on-site outreach programs focusing on health promotion, preventive, curative and case management. Responsible for developing the Patient Navigator/Community Health Worker program and implementing in reach strategies while supervising Patient Navigators, Community Health Workers and the Enrollment team. This role will be responsible for providing quarterly reports, or as requested, showing the impact and growth of the Patient Resources program. Role will oversee the team as they enroll patients in Medicare, Medicaid, the Affordable Care Act, and other supportive service benefits.
Requirements
Minimum qualifications:
Bachelor’s degree in a health-related field, education, communication or social services or equivalent combination of education and experience. Some clinical experience, e.g. EMT, CNA, MA, Social Work or Case Management and Federally Qualified Health Center (FQHC) experience preferred. Must have reliable transportation.

Duties and Responsibilities:
· Assists in providing access to needed services and builds individual, community, and system capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, patient navigation and follow-up, community health education and information, informal counseling, social support, and advocacy.
· Build and maintain a strong with relationship with the Heritage Health care team and collaborative partners to ensure that our outreach efforts increase access to healthcare services and patients are receiving the level of follow-up as identified in outreach registry.
- · Oversee the referral and follow-up process for patients seen at outreach events and mobile health clinics. Provide care coordination and follow-up where needed including documentation in registries and reporting. Documents client encounters and contacts made on behalf of clients; completes and submits quarterly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client. Documents activities, service plans, and outcomes achieved by client in an effective manner.
· Develop, implement and supervise the Patient Resources and Enrollment programs. Provide supervision to Patient Navigators/Community Health Worker and Enrollment teams.
· Assess the need for and implement patient education programs for HHS’s patient population, including adults and children, especially with regard to diabetes, cardiovascular disease, hypertension, cancer, and obesity.
· Provide basic case management as needed to patients as referred by providers. Case management includes but not limited to patient education, establishing self-management goals, referrals services, and collaboration with clinical, pharmacy, nursing, nutritional and social service staff.
· Work with Community Outreach to develop and distribute brochures, posters, and other promotional materials to inform current and potential patients about primary health services at Heritage Health as well as other outreach events.
· Plan and/or participate, in collaboration with Community Outreach, in at least 4 health fairs per year.
· Network with appropriate agencies and inform them of Heritage Health services, as well as receiving information about their services. Keep Heritage Health staff properly informed regarding services available from such agencies.
· Build and maintain a positive relationship with the local schools to facilitate referrals to our clinics. Oversee the outreach coordination between of the Patient Resource team.
· Assess need for new services and outreach methods to maximize outreach by Heritage Health; assist with developing resources for any new projects.
· Continuously expands knowledge and understanding of community resources and services. Facilitates client access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services. Assists clients in utilizing community services, including scheduling appointments with social services agencies and assisting with completion of applications for programs for which they may be eligible.
· Facilitates communication and coordinate services between providers and the clients/patients.
· Works collaboratively and effectively within a team. Establishes positive, supportive relationships with subordinates and peers while providing feedback, correction, and encouragement. Builds and maintains positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Works to reduce cultural and socio-economic barriers between clients and institutions.
· Travels extensively to community locations, various agencies, and other outreach destinations within the Heritage Health service area.
· Coordinates and create a schedule for Patient Resource team at events.
· Oversee Open Enrollment for the ACA as well as Medicare, Medicaid and promotion of Your Health Idaho to our patients.
· Other job-related duties as assigned.
Salary : $24 - $35
