What are the responsibilities and job description for the CCS Coordinator position at Heritage Provider Network?
The CCS Coordinator supports the administrative functions of the Medi-Cal Team. This includes intake, logging, tracking and follow-up for CCS/WCM referrals, and other administrative duties as assigned.
Essential Duties and Responsibilities include the following:
The CCS Coordinator identifies, refers and coordinates care for CCS- and WCM-eligible members and submits referrals to County CCS offices in a timely manner (within 24 hours); ensures referrals are to CCS paneled providers; tracks CCS referrals and their outcomes; assists in meeting regulatory time lines by maintaining an accurate database inventory of benefits, guidelines, programs and member needs; work closely with the Case Management team to coordinate patient care; requests medical records from physicians and hospitals; create job aids; other projects as assigned.
In addition, the position performs data entry, maintains confidentiality when communicating member information and assists with the communication and coordination between programs.

Requirements:Education:
High School Diploma; Associate Degree, Bachelor's Degree or 5 years medical experience.
EXPERIENCE:
- At least one year of experience in Medi-Cal managed care authorization processes.
- Knowledge of Medi-Cal, CCS and WCM programs preferred but not required.
- Knowledge and experience with medical terminology, CPT and ICD-10 coding.
- Microsoft Word, Excel, and Outlook.
