Intake Specialist
40 hrs/week, Mon-Fri, 189 days per year (pro-rated for the remainder of 2022-23)
Location: Alyce Hatch Center, Bend, OR
Rate as established by bargaining agreement, Classified Wage Schedule, starting rate $17.46-$21.25/hr DOE.

Definition
Under direct supervision, the Intake Specialist is responsible for the fixed point of referral for all families, completing intake activities, and conducting developmental screenings. This position requires a high degree of organization in managing a small office, scheduling, entering and maintaining data in a unique database, conducting phone and/or in-person interviews of parents and caregivers, completing developmental screenings, and writing clear and concise summaries.
Qualifications
The Intake Specialist needs a high school diploma and one to two years of advanced education or experience in the specific job requirements. This experience may include relevant work in a similar office environment, or relevant work in an educational setting. Must be able to bend, stoop, and lift up to 50 pounds. Must have access to reliable transportation and consistent attendance is required. Applicants must have successful experience in working with culturally diverse families and communities, and/or have otherwise demonstrated a commitment to equity and strengthening engagement of a diverse community and skill in communicating with a diverse population.

Performance Responsibilities

Complete intake and screening process with parents/guardians of children referred for eligibility evaluations.
- Complete telephone interview.
- Complete developmental screening.
- Schedule evaluation.
- Present child and family information at evaluation planning meeting.
- Enter child information and document all activities in database.

Request pertinent medical and/or educational records.
- Send medical statements for physician to complete and return.
- Complete written report summarizing information gathered through the intake process.
- Work successfully in a team setting with professionals and peers.
- Read, understand, and carry out oral and written instructions.
- Successfully learn the operation of telephones, computers, and other equipment used.
- Communicate clearly and appropriately verbally and in writing.
- Communicate appropriately and regularly through the use of email.
- Respect and maintain confidential information in all situations.
- Fulfill other related duties as assigned.
Terms of Employment
Salary, benefits and vacation are established by policy and collective bargaining agreement.
