MSW Per Diem

Description

Hinds Hospice, one of the most well-known and well-respected nonprofit agencies in California’s Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Per Diem Medical Social Worker to join our team. 

• The Medical Social Worker is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.
• Assesses the psychosocial status of patients and families/caregivers related to the patients terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patients identified residence and assistance when this is not safe and another plan is required.
• Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient and family/caregiver(s) as needed in relationship to stress, and other identified coping difficulties.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
• Provides information to patients, their families/caregivers, and community agencies.
• Serves as liaison between patients, and families/caregivers, and community agencies.
• Assists with bereaved clients in the Center for Grief & Healing for various needs.

Qualifications include but are not limited to:

• Bachelors required, Master's degree (MSW) preferred from a School of Social Work accredited by the Council on Social Work Education.
• Minimum one (1) year of Medical Social Work experience required; Hospice and or Home Health Care preferred.
• Electronic medical record experience preferred.
• Ability to learn and assimilate the Sun Coast Computer Program/System.
• Demonstrated ability to work with health team personnel. Bilingual (English/Spanish) a plus but not required.
• Must have a valid drivers license and a willingness to use personal vehicle in the course of employment.
• Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period).
