Position Summary
The Dental Case Manager provides dental case management for people living with HIV including assisting them in receiving services from the HIV Alliance’s dental program. The Dental Case Manager assists clients in filling out the necessary paper work, organization of paperwork, making appointments, getting to appointments and any follow up necessary in a timely manner. The Dental Case Manager also builds and maintains community partner relationships with local dental clinics.

Essential Functions and Responsibilities
Maintain an individual client load as Dental Case Manager.
Prioritize client dental needs in a timely manner.
Problem solving with clients.
Maintain and organize client forms and charts.
Maintain HIPPA compliant confidentiality standards in all communications.
Keep up to date with HIV/AIDS disease and treatment issues.
Provide direct on-on-one phone care coordination services.
Visit accompaniment services.
Act as primary contact for dental providers and community partners.
Process referrals, including follow up and documentation.
Advocate with other case managers and other service providers on behalf of clients.
Keep up-to-date record of client appointment attendance and problem solve any attendance barriers.
Build and maintain a positive working relationship representing the agency with community partners.
Develop, maintain and strengthen relationships with service providers.
Maintain knowledge of Medicaid dental benefits and how to access benefits.
Maintain up-to-date and accurate client records, including CareWare.
Participate in necessary meetings.
Assist Pilot Programs Manager with all necessary reports.
Participate in agency teams.
Other duties as defined by Pilot Programs Manager or Program Director.

Required Qualifications
Minimum one year of client-centered service experience
Excellent written and oral communication skills
Experience with Word, Excel and Access
Self-motivated/self-managed and detail-oriented
Ability to work with diverse populations
Record keeping and organizational skills
Team player with high-energy and enthusiasm
Valid Oregon Driver’s license
TB test (provided at agency)
Ability to pass agency criminal background check

Preferred Qualifications
Case Management experience (with clients who have substance abuse and/or mental health issues).
Experience coordinating medical and/or dental care.
Experience working with drug and alcohol, mental health issues

Physical Demands
While performing the duties of this job, the employee is required

  • Repetitive use of hands such as grasping, twisting/turning of wrists, and finger dexterity necessary to perform various clerical duties, such as typing and writing.
  • Frequent use of arms such as pushing and pulling to open and close drawers and cabinets
  • Ability to sit or stand for extended periods of time at an assigned station
  • Ability to bend, stand, and reach to be able to transport clients and to occasionally lift and carry up to 25 pounds (harm reduction kits, naloxone kits, testing and education materials)
  • Continuous clarity of vision at 20 inches or less for processing of paperwork

Working conditions, Travel and Environment
The duties of this job require individuals be able to work in non-traditional and community-based settings
The duties of this job require regular attendance

  • Ability to work flexible hours, including early mornings, evenings, and weekends
  • Ability to work 5 days per week for a minimum of 40 hours per week

Travel to other locations as necessary to complete essential job functions

  • Ability to transport self to job-related events and locations
