The HCP of Texas Registered Nurse Case Manager is responsible for the admission, planning, intervention, and ongoing evaluation of patients in home care. RN Case Manager must have expert level clinical assessment skills and make independent decisions for the well being of the patient.
Knowledge of the OASIS and be able to complete home visits as assigned.
RN Case Manager will partner with an LVN and have an assigned territory within a manageable proximity.
