Homestead Village is mid-sized retirement community conveniently located just outside Lancaster City. We are looking for an experienced Assistant Director of Nursing/Registered Nurse Assessment Coordinator to join the Homestead Village team. The ideal candidate will have the knowledge and capability to function effectively as the RNAC (Registered Nurse Assessment Coordinator) for a 58 bed Skilled Nursing Facility. We are looking for a hard-working, skillful, and compassionate registered nurse who has also has developed good leadership and administrative abilities.
Qualifications:
- Current PA Registered Nurse license: Bachelor’s degree in nursing, preferred
- Previous RNAC exp preferred or previous exp in related position.
- CPR Certified or ability to obtain.
- Excellent computer skills, including EMR & MDS.
- Knowledge of LTC regs & operations, including Federal, State, & Local regulations.
- Demonstrated leadership and supervisory skills.
- Effective & professionally communicate verbally/writing with all stakeholders.
- Ability to effectively educate & train co-workers on clinical and non-clinical topics.
- Knowledge of infection control regulations and procedures.
- Knowledge of wound prevention, assessment, and treatment.
- Adheres to the Company Wide Behaviors
Responsibilities (the following is an overview of the responsibilities of the position and should not be considered to be all inclusive):
- Assist and support fellow nurses in coordinating all nursing services in accordance with SNF Rules & Regulations, Resident Bill of Rights, Nursing Services P&P, and the HV Employee Handbook
- Provide supervision to staff through effective communication, education, coaching and corrective action
- Co-Manage Wound Care and Infection Control services in SNF.
- Co-Manage MDS Scheduling and Coordination with Medicare A (small), Medicaid, and Private Pay population.
- Update care plans quarterly and as needed, follow up on care plan, monitor restorative nursing program completion.
- Maintain 672 and 802 forms for assigned households.
- Participate in the On-Call rotation.
